REQUIREMENTS PhilHealth benefits may be used when proper requirements have been complied with the PhilHealth Section prior to discharge. Failure to submit all the requirements would mean no PhilHealth deduction will be less in the final bill. However, you can still file your benefit claim within 60 days after discharged directly to PhilHealth office. We would like to reiterate that your PhilHealth form should be properly filled up by the Attending Physician and your Employer and filed with our PhilHealth Section at least 2 days before date of discharge to facilitate processing PhilHealth

Arrangement can be made through the Emergency Room. YOUR BELONGINGS What to Bring: 1. You may bring personal effects and items you will need during your confinement at BSJH. Patients admitted to rooms are already provided with an admission kit, which contains: - Thermometer - Calibrated drinking glass - Spoon and Fork - Face Towel - Tissue paper - Rubbing alcohol 2. You will be provided with hospital gowns at the Nurses’ Station. However you may bring your own pajamas. What not to Bring: For your safety and security, we discourage you to bring large amounts of money, jewelry and other valuable personal items. The Hospital Management also discouraged patients and relatives not to bring any electronic appliances to avoid loss or can cause fire. NEVER LEAVE YOUR THINGS UNATTENDED THE HOSPITAL WILL NOT BE RESPONSIBLE FOR LOST OR STOLEN VALUABLES. We advice you to lock your room, whenever you leave for examination or consultations. If you have inquiries or urgent need from our maintenance or housekeeping staff, we encourage you to transact only with those wearing the prescribed BSJH ID. Billing Department provides patients with a notice for initial payments and a running bill or a partial bill. We advise that you make arrangements to pay regularly in order that the bill does not accumulate. The balance of accounts if any should be paid upon discharge. Professional fees of your Attending Physician are billed together with the hospital bill and must be paid in cash at the Cashier. In cases of HMO or Company Insurance cardholders, only those hospital charges, only those procedures covered under the Letter of Authority can be charged to the HMO or Company Insurance, the rest must be settled through cash only at the Cashier Section. VISITING PRIVILEGES Rest is very important to hospital patients. Hence, the following visiting hours have been prescribed for your convenience and comfort. A maximum of two companions are allowed per patient admitted in regular rooms. Children below 7 years old are advised not to visit patients to avoid possible acquisition of virus or infection. Your Responsibilities as a Patient As a patient at Bataan St. Joseph Hospital, you and/or your representative are expected to: • Provide complete and accurate information about your health, including present condition, past illnesses and hospitalizations, medications, natural products and vitamins, and any other matter that pertains to your health. • Provide complete and accurate information including your full name, address, home telephone number, date of birth, insurance carrier, and employer, when it is necessary. • Provide your doctor or the hospital with a copy of your advance directive if you have one and if you want it to apply during your admission. • Ask questions when you do not understand what your doctor or other member of your health care team will tell you about your diagnosis and treatment. You should inform your doctor if you anticipate problems in following prescribed treatment. Inform your doctor if you are considering alternative therapies. • Ask your doctor or nurse what to expect regarding pain and pain management plan. You should tell your doctor or nurse about any worries you have about taking pain medication. • Leave valuables at home and bring only those items that you need during your hospital stay. • Abide by all hospital rules and regulations. • Comply with the NO SMOKING policy. • Comply with Visiting Hours and Policies to ensure the rights and comfort of the other patients, as well. Be considerate of noise levels, privacy and safety. Weapons are prohibited inside hospital premises. • Treat hospital staff, other patients, and visitors with courtesy and respect. • Provide complete and accurate information for insurance claims and work with the hospital’s Credit and Collection Staff to ensure proper payment settlement. • Know your health insurance coverage and related policies including the hospital and doctors covered by your insurance provider and its policies about required pre-approvals, covered services and admissions. • Pay your hospital and physician bills in a timely manner. • Ask questions of your insurance company or the hospital’s Billing Services if there is a financial issue that you do not understand.
