Your Attending Physician will tell you when you are well enough to leave and will order your discharge. After you have settled your accounts, the Cashier will give your clearance slip, which will be presented, to the Nurse on Duty or Guard on Duty. To avoid delay in the discharging process, kindly submit all applications for PhilHealth and/or Senior Citizens’ discount at least 2 days before your discharge date at the Accounting Office. Patients are advised to wait for a call in your room (thru the Nurses Station) from the Cashier Section of the Accounting Office when to settle your hospital bill. Discharging Procedures

Your Rights As a Patient • Receive considerate and respectful care and treatment in a clean and safe environment free of unnecessary restraints and without discrimination with regards to race, color, religion, sex, national origin, disability, sexual orientation or source of payment. • Receive emergency care if you need it. • Receive care in a safe environment free from all forms of abuse, neglect or harassment. • Receive complete information about your diagnosis, treatment and prognosis in easy-to-understand terms. If it is medically inadvisable for you to receive this information, you may designate your legal representative for this purpose. • Participate in decisions about your treatment and discharge from the hospital. • Give written informed consent before any non-emergency procedure begins. • Have your pain assessed and to be involved in decisions about managing your pain. • Be informed of the names, positions and functions of any doctor or hospital staff in charge of, or involved in your care. • Refuse treatment and be informed of the effects and consequences of refusal. You can refuse treatment, examination or observation by a particular health care practitioner, and can request for their replacement. We will inform you about the consequences of treatment refusal, or of replacing a doctor or staff member, and will provide a replacement. • Receive all the information you need to give informed consent for. This includes proposed procedures or treatments that need informed consent, including possible risks and benefits, participation in research, and orders not to resuscitate. • Designate a legal representative to receive the information and give consent if you are too ill or if it is medically inadvisable for you to receive the information and make the decision. • Receive, free of charge, a written discharge plan and description of your medications. • Privacy while in the hospital and confidentiality of all your information and records. • Receive an itemized bill, including an explanation of all charges. • Complain-without fear of reprisal-about the care and services you receive. • Authorize family members and other adults who will receive priority to visit, consistent with your ability to receive visitors. REQUIREMENTS PhilHealth benefits may be used when proper requirements have been complied with the PhilHealth Section prior to discharge. Failure to submit all the requirements would mean no PhilHealth deduction will be less in the final bill. However, you can still file your benefit claim within 60 days after discharged directly to PhilHealth office. We would like to reiterate that your PhilHealth form should be properly filled up by the Attending Physician and your Employer and filed with our PhilHealth Section at least 2 days before date of discharge to facilitate processing PhilHealth

Arrangement can be made through the Emergency Room. YOUR BELONGINGS What to Bring: 1. You may bring personal effects and items you will need during your confinement at BSJH. Patients admitted to rooms are already provided with an admission kit, which contains: - Thermometer - Calibrated drinking glass - Spoon and Fork - Face Towel - Tissue paper - Rubbing alcohol 2. You will be provided with hospital gowns at the Nurses’ Station. However you may bring your own pajamas. What not to Bring: For your safety and security, we discourage you to bring large amounts of money, jewelry and other valuable personal items. The Hospital Management also discouraged patients and relatives not to bring any electronic appliances to avoid loss or can cause fire. NEVER LEAVE YOUR THINGS UNATTENDED THE HOSPITAL WILL NOT BE RESPONSIBLE FOR LOST OR STOLEN VALUABLES. We advice you to lock your room, whenever you leave for examination or consultations. If you have inquiries or urgent need from our maintenance or housekeeping staff, we encourage you to transact only with those wearing the prescribed BSJH ID. Mass is celebrated at the forth floor of the main hospital every Sunday at 11:00 am. The sacraments as well as visitation and counseling of the sick are done Monday to Friday, Saturday and Sunday for request only. You may also request through the Nurse on Duty. Pastoral / Chapel

1. EMERGENCY ROOM ADMISSION Emergency admission should be coursed through the emergency room. An identification bracelet will be placed on your wrist with your name and that of your Attending Physician. Please do not remove this ID band until after you have been discharge since our staff will always refer to this when administering tests and prescribed medication. 2. DIRECT TO ROOM All patients from direct room admission must have admitting order from the Attending Physician. The Admitting Clerk will endorse the patient to the Nurse on Duty once the room is prepared. Admission
